Pain therapy does not consist only in relieving pain regardless of the cause, setting the therapeutic choices on the evaluation of its intensity. On this premise, the main pillar of this book is that with the algological evaluation we arrive at the pathogenetic diagnosis of pain and that the therapy is based on it. The reasoning that supports this way of proceeding can be summarized as follows: “... having found that pain is produced by this particular pathogenetic mechanism by configuring this particular pathogenetc type, I choose this therapy because, by acting in this particular way, it is able to counteract that mechanism…”. This reasoning forces us to consider the reasons for therapeutic choices each time and to ask ourselves each time which drug or procedure may work or not work and why.
